Working Together For Your Care event - Wednesday 25 April 2018
We recognise that care for the members of our community who are over 65 is not always joined-up or easy to access. We also recognise that people would like more independence around managing their health and care. As such, Health and Care teams and the community/voluntary sector in Harrow are working together to develop options for an improved and joined-up health and care service for our over 65s. We have agreed to focus initially on our most frail older people including those in care homes, those with dementia and adults needing palliative care. By contrast we also have a mostly healthy 65+ group to ensure we have measures for keeping people healthy and independent.
We held a clinical summit on 31 January 2018 and a public engagement event on 7 February 2018 to ask for your thoughts on what a good service would look like. These events were well attended and gave us valuable information to help us in developing the improved service. We would like to invite you to the next in that series of events on Wednesday 25 April to feed back on the progress from these meetings, test proposed outcomes and also to reach out to more members of the public/ carers and front line staff around the care of our older population.
